The EDOF lens: continuous vision from distance to intermediate
Among the types of intraocular lenses, the EDOF lens sits midway between the monofocal and the trifocal. Its name comes from Extended Depth of Focus, and it sums up well what it does: instead of a single focal point, it offers a continuous range of vision that runs from distance to the intermediate range. In this article I explain how it works, what it offers and what it asks in return, and which patients it suits best.
How an EDOF lens works
The key difference from other lenses is how they distribute light. A monofocal has a single focus; a trifocal creates three separate foci (far, intermediate and near). The EDOF lens, by contrast, does not create several points: it stretches that single focus into a continuous range. The result is vision without “jumps” between distances —from far to the computer or the car dashboard, everything is focused progressively.
That continuity is its great value: the transition between distances is natural and the brain adapts to it easily.

Advantages
- Continuous far and intermediate vision. Very comfortable for driving, cooking, and looking at the computer or the phone at a normal distance.
- Fewer night-time phenomena than the trifocal. By not splitting the light as much, night halos are considerably lower —especially in the more modern refractive designs—, which makes it kinder for night driving.
- Good contrast quality. It comes closer to the sharpness of a monofocal than a trifocal does.
- Forgiving. It tolerates small deviations better and usually adapts well.
Disadvantages
The main trade-off is near vision. The EDOF lens gives very good far and intermediate vision, but near reading is more limited: for small print, the phone in low light or long reading sessions, glasses are often still needed. It is therefore not the lens of complete freedom for near vision —that role belongs to the trifocal.
Types of EDOF lenses
Not all EDOF lenses are achieved in the same way. There are two main families:
- Diffractive: they use micro-rings that stretch the focus. They offer a little more range, but can produce some night halo, though less intense than a trifocal.
- Refractive (ring-free): they shape the light through the form of the lens, without diffractive elements. They tend to produce fewer halos and very good contrast. The J&J TECNIS PureSee is an example: a purely refractive EDOF designed for a continuous range of vision with a very low incidence of night-time phenomena.
Who is it the first choice for?
The EDOF lens is an excellent choice for several profiles:
- Active people who want freedom from glasses for far and intermediate (driving, screens, daily life) and who accept using glasses occasionally for small print.
- Night or professional drivers who want more autonomy than a monofocal but without the halos of a trifocal: here the EDOF, especially the refractive one, is the sweet spot.
- Those who want to take a step up from the monofocal without fully taking on the trifocal’s night-time phenomena.
- Certain eyes with a mild condition where a trifocal would be too risky but a little more range is desired; some EDOF designs are more forgiving.
And the halos?
Night-time phenomena can appear with any lens that extends the focus, but with EDOF lenses they are much more discreet than with trifocals, and almost imperceptible in the refractive designs. The EDOF lens is the lens of balance: it offers continuous, natural vision from far to intermediate, with high optical quality and few night-time phenomena, in exchange for more moderate near vision. It is not the one that gives the most freedom from glasses —that is the trifocal— but it is one of the most balanced and safe for those who prioritise visual quality and night-time comfort.
